The Collegiate Sports Medicine Foundation is dedicated to understanding the health care of the collegiate student athlete as well as the university health care professional.  If you have an idea or a question you would like to see addressed contact Brian Anderson at banderson@cmsfoundaiton.org.

The below surveys are completely anonymous, thus no names will be released in the results.   It is important from time to time to understand the classification of the institution submitting information.
