PPE Reimbursement Program Information Page

Thank you for signing up for one of our online demonstations as to how th program works.  We are excitied about the opportunity to partner with you to generate revenue for your sports medicine program.  For us to maximize the discussion as well as your revenue, we are going to need a few items.  It is not required to complete these prior to the session however it will be helpful to have the information at hand.

Items that we would like to see.

1.  Complete the PPE Reimbursement Information Form and fax back to 404-835-4069.

2.  Provide us a blank HIPAA / Privacy Form that you would use for your student athletes.

3.  Copy of the actual physical form a physician would use in the PPE.

4.  Copy of the medical history you use for your student athletes.
