Cost:

Interior Room: $650.00 / person (Port Fees and Tax Included) based on double occupancy until December 1, 2007)
3rd and 4th Peron = $375 each
Ocean View Room:  $750.00 / person (Port Fees and Tax Included) based on double occupancy until December 1, 2007)
3rd and 4th Person = $415 each
Balcony Room:  $975 / person (Port Fees and Tax Included based on double occupancy until December 1, 2007)
3rd and 4th Person = $485 each

Meeting Attendance Fee:
For each person attending the meeting there will be a $50 assessment fee.

Example:  If a husband and wife were to occupy a Interior Cabin with one of them attending the meeting the total cost would be $685 x 2 + $50 = $1350.  If both were attending the meeting the total cost would be $1400.

Deposit Plan:

Interior Room: $350 / person (Based on double occupancy) by December 1, 2007 and $300 (Balance) by Feb. 10, 2008

Ocean View Room:  $450 / person (Based on double occupancy) by December 1, 2007 and $300 (Balance) by Feb. 10, 2008

Balcony Room: $500 / person (Based on double occupancy) by December 1, 2007 and $475 (Balance) by Feb 10, 2008

Note:  Each room will receive $100 in on board credit to be used for excursions, gaming, refreshments, transfers, tips, etc.,  For those requesting room for more than two individuals please contact Brian Anderson directly.

If you would be interested in presenting on a topic that is relevant to the Collegiate Sports Medicine Foundation please contact Brian Anderson at 954-941-8822 or banderson@csmfoundation.org.  Topics are 20 min. of presentation with 10 min. for discussion.  The presenter will receive an additional 10 CEUs.  Such topics could but not limited to the following:  Appropriate Medical Coverage, MRSA, PPEs, Sickle Cell, Asthmatic, Emergency Action Planning, AEDs, Heat and Hydration Management Strategies, Concussion Management, Eating Disorder Management, Physician Staffing, Cost Containment Strategies, Etc.,

The meeting will contain 15 CEUs plus an additionally 10 Additional CEUs if you wish to present a topic.
